Join Our Care Coordination Team! MSW Social Worker Opportunity

Location : Chilton Medical Center, Pompton Plains, NJ

Position : MSW Social Worker

Start Date : May 12, 2025

End Date : August 15, 2025

Schedule : Monday – Friday, 8 : 30 AM – 4 : 30 PM (Weekends as needed)

Are you a compassionate and culturally competent social worker ready to make a lasting impact on patients' lives?We areseeking a dedicated MSW Social Worker to join our dynamic Care Coordination team. In this role, you’ll collaborate with a multidisciplinary team to provide patient-centered care and address social determinants of health (SDOH). Be part of an essential support system that guides patients through their healthcare journey while ensuring safe transitions and wellness!

Key Responsibilities :

  • 60% – Conducting patient assessments, making referrals, and delivering interventions to ensure care needs are met.
  • 20% – Documenting interactions and maintaining thorough, accurate records.
  • 10% – Participating in meetings, trainings, and contributing to program development.
  • 10% – Collaborating with internal and external agencies to improve patient care coordination.

    • What You’ll Bring :

  • Experience : 3–5 years of experience in a health-related field (hospital experience is a plus!)
  • Skills : Strong understanding of social services and community resources, with excellent organizational, communication, and interpersonal skills.
  • Knowledge : Familiarity with Epic EMR preferred; proficiency in Microsoft Office and EMR documentation standards.
  • Licensure : Active LSW license in New Jersey (in good standing).
  • Bonus : Bilingual skills and experience in behavioral health are a plus!
