Registered Nurse Case Manager Hospice

We are hiring for a Registered Nurse Case Manager in Hospice.

This is a Full Time position doing Home Visits for Hospice care in the Danville, VA area.

• Pay range $34-$38 / hr
• Service area includes Pittsylvania county and occasionally Henry county.

The Registered Nurse Case Manager assumes full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies / procedures / goals / objectives of hospice to assure quality patient care.
