Responsibilities

The Supportive Care / Palliative Care Nurse Practitioner (SPCNP) collaborates with the HarmonyCares Medical Group staff to deliver comprehensive, high-quality palliative care services under the supervision of the Regional CMO. The SPCNP will provide patient visits to both new and established palliative care patients, coordinating their care plans with the designated team, including home visits, facility visits, and hospital consultations. Engaging in independent decision-making regarding patient healthcare concerns and goals of care is a key aspect of this role.

Essential Duties and Responsibilities

  • Provide medical expertise on pain management, symptom management, and prognostic evaluations for patients and their families, including :
  • Ordering necessary therapies and tests
  • Conducting in-person and / or telehealth visits as needed
  • Focus on patient experience and identify patient goals of care
  • Ensure care aligns with established palliative care guidelines and standards
  • Regularly communicate with HarmonyCares Medical Group regarding new and existing patient referrals
  • Coordinate care with external primary care providers and agencies as needed
  • Collaborate with attending physicians on patient care and treatment plans
  • Perform patient visits in private residences, long-term care facilities, assisted living facilities, hospitals, or other locations
  • Provide expertise in pharmacy formulary and medication management
  • Collaborate with the Chief Medical Officer and Executive Director
  • Independently commute to patients' locations for visits and attend meetings as necessary

    • Qualifications

    Required Knowledge, Skills, and Experience

  • Active and unrestricted state licensure as an Advanced Practice Nurse
  • Valid NPI#, DEA, and CTP
  • Enrollment with Medicare or state Medicaid, or eligibility for enrollment
  • 3 years of experience in home-based care or hospice care
