A company is looking for a Community Connector, Rural at Heart.

Key Responsibilities

Implement evidence-based blood pressure control practices and provide related education

Support community partner capacity to improve health outcomes and implement nutrition security initiatives

Recruit, train, and coordinate volunteers and community partners to achieve community impact goals

Required Qualifications

High school diploma or equivalent required; college coursework in public health preferred

Proven track record in communications and organizational skills

Commitment to improving rural health outcomes

Intermediate knowledge of Microsoft Outlook and Word

Able to lift and / or move large objects and access reliable internet when working remotely
