Collaborates with medical providers and interdisciplinary team members to establish individualized medical goals to maximize healthcare outcomes. Coordinates multi-disciplinary care conferences for high-risk or complex patients. Manages acute and chronic care needs as well as health maintenance by reviewing reminders and triggers and providing outreach and education to patients. Participates in case management reviews with a multi-disciplinary team of providers. Acts as an advocate for patient and family decision-making and self-care. Utilizes problem-solving skills in meeting the needs of patients and providers. Facilitates the coordination of patient referrals and tracks the progress, providing periodic status reports to providers. Identifies delays and obstacles to care and works to remedy them. Performs various nursing duties including assessment, documentation, education, and administering medications and therapies. Maintains annual competencies and adheres to safety and infection control policies. Communicates collaboratively with team members within departments. Participates in team improvement efforts and the development of the case management role. Acts as a triage nurse and responds to emergency situations. Performs additional duties as assigned. Facilitates the coordination of specialty clinic referrals and manages waiting lists. Ensures appropriate diagnostics are performed before appointments and manages the clinical space for specialty clinics. Tracks referrals and outcomes of care, providing status reports to providers. As primary case manager for OB patients, is responsible for intake, tracking, and outreach. Coordinates video teleconferencing visits with specialty providers. Manages referrals for exercise treadmill tests and coordinates appointment times.
