Med-Care Providers delivers compassionate, high-quality care to patients in the comfort of their homes. We are expanding our specialty services and seeking a Nurse Practitioner with experience in Pain Management to conduct in-home visits and provide personalized, integrative pain care to seniors and medically complex patients.
Position Overview :
We are looking for a skilled and empathetic NP who can assess, diagnose, and manage chronic and acute pain through in-home consultations. The ideal candidate will prioritize patient safety and evidence-based approaches, while coordinating care with primary providers and specialists.
Responsibilities :
- Conduct home visits for patients requiring pain management
- Perform comprehensive pain assessments and develop individualized care plans
- Prescribe appropriate therapies including medications, injections, and non-pharmacological approaches
- Monitor treatment effectiveness and adjust care plans accordingly
- Educate patients and caregivers on pain management strategies and safe medication use
- Maintain clear documentation in the EMR system
- Collaborate with physicians, specialists, and interdisciplinary teams
