$40,000 Student Loan Repayment or $25,000 Sign-on Bonus for individuals who have not previously participated in this program Optum is seeking a Statewide Nurse Practitioner - Traveler to join our HouseCalls team in Iowa. This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being. Growing together. This role requires travel across the entire state of Iowa, approximately 85% of the time. All travel expenses are covered, and there is likewise a base salary plus multiple incentives. Primary Responsibilities : Conduct in-home assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam Perform evidence-based practice screenings including point of care testing (as appropriate) Identify diagnoses to be used in care management and active medical management in the furtherance of treatment Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment Communicate findings in your assessment that will be used to inform member's PCP of potential gaps in care Identify urgent and emergent situations and intervene appropriately Educate members on topics such as disease process, medication, and compliance You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications : Completion of Nurse Practitioner accredited program Current NP license Current state RN license Driver's license and access to reliable transportation; if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy Current valid DEA license or ability to obtain by start date Frequent Travel across the entire state of Iowa, approximately 85% of the time Preferred Qualifications : 2 years of clinical experience, primary care, home-based medical care, geriatrics or palliative care or emergency / urgent care Knowledge of CPT, ICD-10 and HCPC codes Knowledge of clinical standards of care Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems Awareness of NCQA requirements, CMS guidelines, Medicaid / Medicare contracts and benefit systems Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
