- 8 : 00 AM - 5 : 00 PM

Travel Required : Yes - 100% travel between senior living facilities during shift
Position Summary
Seeking an Advanced Practice Clinician (APC) to provide integrated, longitudinal care to high-risk health plan members in senior living facilities. This role offers flexible scheduling with no on-call, weekend, or holiday requirements, and is part of an interdisciplinary team focused on improving outcomes for patients with complex medical and behavioral needs.
Primary Responsibilities
Conduct comprehensive patient assessmentsManage acute and chronic medical and behavioral conditionsEnsure accurate documentation, including ICD-10 codingCollaborate with interdisciplinary care teamsLead advanced illness and care planning conversationsProvide education and counseling to patients and caregiversRequired Qualifications
Active and unrestricted NP or PA license in the state of practice (or ability to obtain by start date)NP : Graduate of an accredited MSN or DNP programPA : Graduate of an accredited PA programBoard certification :NP : Certified by AANP or ANCC in Family, Adult, Gerontology, or Adult-Gerontology Acute CarePA : Certified by NCCPACurrent DEA license and prescriptive authorityAbility to meet physical requirements (e.g., lifting 30 lbs., navigating stairs, kneeling for assessments)Reliable transportationAbility to obtain collaborative practice agreement if required by stateFull COVID-19 vaccination (per federal, state, and client requirements)Drug screening required prior to employmentPreferred Qualifications
1+ year of clinical experience (long-term care preferred)Experience with complex behavioral, social, or functional patient needsUnderstanding of geriatrics, chronic illness, and end-of-life careProficiency in electronic medical records and documentation
