Key Responsibilities

Conducts patient outreach to facilitate post-discharge care and engagement

Collaborates with the nursing team to enhance care coordination and address barriers

Maintains quality data and tracking systems for patient care and reporting

Required Qualifications

High School Diploma / GED or Specialized Diploma in Medical Assistant or related field

3 years experience in a complex health system environment or related field

Proficient in Microsoft Office applications

Knowledge of medical terminology and electronic medical records

Ability to handle multiple tasks and maintain confidentiality
