Description :

Home Health Services provide nursing care to patients in their homes.

Typical caseload involves seeing an average of 6-7 patients per day, totaling 30 patients per week.

Admits are weighted at 2.40 units with an expectation of completing about 2 admissions per day.

Candidates are expected to have at least 1 year of Home Health experience.

First-time travelers with Home Health experience as a nurse will be considered.

Patient demographic includes adults aged 18 and older, with common diagnoses including wound care, Pleurx drains, catheters, feeding tubes, diabetes management, heart failure, and other medical management.

Travel within specified areas is required.

Skills required include Oasis knowledge, hospice philosophy, infection control practices, wound vacs, wound care, PICC line dressing changes, port access, IV infusions, drains, and critical thinking.

Candidates must be able to drive at night.

Support within the department includes leadership and peers.

Technological equipment includes EMR systems, mobile phones, and discharge instructions uploads.

Scheduling requires manager approval for requests.

Scrub color is navy.
