IV certification (required) and BLS certification (required). 1+ years of experience in home health care (or similar environment). Wound care expertise is a must. Ability to assist with patient mobility and transfers. Access to reliable transportation and a valid driver's license. Proof of insurance for your vehicle.

What You'll Be Doing :
- Conducting patient assessments (start-of-care, post-hospital, and reassessments) with a focus on personalized, patient-centered care.
- Creating, implementing, and evaluating care plans alongside physicians, caregivers, and the patient.
- Using OASIS data for ongoing assessments, ensuring optimal care at every visit.
- Providing expert wound care and other nursing treatments to patients.
- Maintaining accurate, timely documentation via CradleMRx software.
- Complying with all health and regulatory standards.
