Call Ratio : 50%

Average Number of Cases Per Day : 10

Documentation System / EMR : Epic

Trauma Level / Designation : Level 1

Annual ER Volume : 250 K

Number of Operating Rooms : 2

Description of Ancillary Staff : full compliment

Hemodialysis Access : Yes

Patient Age Demographic : Adult and geriatric

Vascular Surgery Procedures Required :

Aneurysm repairs, Thoracic, Abdominal, Peripheral, Vascular Access Devices, AV shunt

Endovascular Surgery Procedures Required :

Carotid Artery Stenting, Bypass Surgery, Aortofemoral Bypass, Stenting, Endovascular and Open Surgical Reconstruction for Deep Vein Occlusions

Treatment of : Lymphedema

Endovascular Therapy Required :

Peripheral Artery Disease, Gangrene of the limbs

Facility Reimburse for the Following Costs : Mileage Billed at the IRS Standard Rate, Rental Car, Hotel, Flight

Requirement description :

• Board Certified / FACS - [ REQUIRED]
• Active OH license (no exceptions) - [ REQUIRED]
• DEA - [ REQUIRED]
• BLS / ACLS - [ REQUIRED]
• Provider must be available for weekend coverage - [ REQUIRED]
