Schedule & Staffing
Shift : Day shift (8-hour shifts typical)
Schedule : Varies based on unit needs
Schedule issued in 4-week blocks
Weekend Coverage : Rotational (required)
Call Required : Yes
Floating Required : Yes
Staffing Ratio : 1 Case Manager : up to 25 patients
Experience Requirements (HARD STOPS)
Minimum 2 years of Acute Care Case Management experience REQUIRED
Must have experience performing Care Coordination and Discharge Planning
Travel experience REQUIRED (first-time travelers NOT accepted)
Epic experience REQUIRED (HARD STOP)
Role Scope (IMPORTANT)
Case Managers DO NOT perform Utilization Review
This role is focused on :
Care coordination
Discharge planning
Patient and family support
Required Certifications
BLS Certification
Required Core Responsibilities
Assess patients and develop individualized discharge plans
Perform care coordination across the continuum
Organize and participate in family meetings
Collaborate in development of treatment plans
Arrange post-acute services independently, including :
DME
SNF placement
Ambulance transportation
Coordinate with : Physicians
Nursing staff
Social services
External providers and payors
Maintain accurate, timely documentation in Epic
Ensure compliance with HIPAA and regulatory standards
Required Knowledge & Skills
Acute care hospital case management workflows
Experience with HMOs, IPAs, and managed care plans
Discharge planning for complex social situations, including :
Homelessness
Mental health history or crisis
Substance abuse treatment options
Adoption / foster care processes
Advance directives & POLST
Protective services processes
Strong communication and organizational skills
Ability to manage a high-volume caseload independently
Preferred Experience
Community hospital experience
Familiarity with regulatory standards (CMS, Joint Commission, HIPAA)
