Monday-Friday with flexibility during daytime hours, on-call as assigned Coverage area for this position will be Leavenworth and Douglas County and parts of Kansas City, Kansas Ability to work one on one with patients Work autonomously as a RN Performs nursing assessments in the patients' home Utilizes full scope of nursing skills, including wound and ostomy care, PICC line care, chronic care education and medication reviews Coordinates patients' care - consulting other disciplines of the health care team Individualizes patient care plans and adjusting or modifying the nursing plan of care as appropriate Collaborates with the medical staff and the other disciplines to provide and enhance patient care Missouri and Kansas RN License Required Why Saint Luke's?
