What You'll Do

As an Admissions Registered Nurse, you will :

• Assess Patient Needs : Identify and evaluate the physical, psychological, social, and spiritual needs of patients and their families.
• Develop Care Plans : Initiate appropriate interventions and create comprehensive, responsive care plans tailored to each patient's unique needs.
• Coordinate Care : Collaborate with the interdisciplinary team (IDT) to ensure seamless delivery of services to patients and families.
• Educate Caregivers : Provide education to patients, families, and primary caregivers on effective care techniques and best practices.
• Advocate for Patients : Maintain regular communication with attending physicians, long-term care facility staff, HMO case managers, and other stakeholders to advocate for the patient's needs and preferences.
