Schedule : Full Time Sunday - Thurs. 9am - 5 : 30pm
Summary
The Connections Program is The Kensington’s signature early-stage memory care program, designed to support residents experiencing mild cognitive change while still maintaining a high level of independence, engagement, and purpose. The program bridges the gap between traditional Assisted Living and Memory Care—providing structured cognitive stimulation, social connection, physical wellness, and personalized support to help residents thrive.
The Connections Program Manager is responsible for developing, coordinating, and leading this specialized program. This role ensures that daily programming, resident support, family communication, and team collaboration are aligned to the program’s mission : to enrich quality of life, promote cognitive strength, and foster meaningful moments for each resident.
Duties & Responsibilities
Develop, implement, and oversee a robust calendar of daily Connections programming focused on cognitive stimulation, social engagement, emotional wellness, and physical activity.
Maintain a structured daily schedule that provides predictability, purpose, and appropriate cognitive challenge for residents.
Lead small and large group sessions, one-on-one engagements, and specialized activities tailored to individual needs and abilities.
Evaluate program effectiveness regularly and adjust offerings based on resident outcomes and team feedback.
Complete resident assessments and assist with the creation of personalized support plans that address cognitive, social, and wellness needs.
Monitor resident progress, participation, and changes in condition, providing proactive communication to families and appropriate team members.
Document daily attendance, engagement notes, and any significant observations in accordance with community standards.
Maintain strong communication with families, including regular updates, touchpoints, and participation in care-plan discussions.
Host or support family education workshops, social events, and Connections-specific gatherings.
Serve as an ambassador of the program during tours, open houses, and marketing events.
Collaborate closely with Assisted Living leadership, Life Enrichment, Clinical team members, and Dining Services to ensure a holistic resident experience.
Provide coaching, guidance, and support to team members participating in the Connections neighborhood.
Participate in interdisciplinary meetings, service plan reviews, and training initiatives.
Maintain organized program spaces that support engagement, safety, and calm sensory environments.
Oversee supply inventory and ensure materials are appropriate, clean, and prepared for activities.
Ensure compliance with all state regulations and internal policies related to programming, documentation, and resident care.
Knowledge & Skill
Strong communication and interpersonal skills, with the ability to motivate, engage, and support residents with cognitive change.
Experience leading creative programming such as brain fitness, wellness classes, discussion groups, music, art, and sensory-based activities.
Ability to observe resident needs and adapt activities in real time.
Strong organizational and time management skills.
Proficiency with computers and documentation platforms (Microsoft Office, electronic health records, etc.).
Qualifications
Experience working with older adults or individuals with cognitive impairment required.
Prior experience in Life Enrichment, Memory Care, recreation therapy, social work, or a related field strongly preferred.
Ability to read, write, and communicate clearly in English.
Must maintain a professional appearance in accordance with the community dress code.
Ability to work flexible hours, including occasional evenings, weekends, and holidays for events or resident needs.
Must pass all required health screenings and criminal background checks per state regulations.
Physical Requirements
Ability to stand, walk, sit, bend, reach, and lift up to 25 lbs.
Must be able to lead activities requiring movement and assist residents when needed.
Must be able to remain calm, positive, and solution-focused in a dynamic environment.
