Medical Director – PACE Program | Mishawaka, IN | $231K–$280K | Geriatric Leadership Role

Location : Mishawaka, Indiana (near South Bend & 80 miles from Chicago)

Setting : Program of All-Inclusive Care for the Elderly (PACE)

Focus : Frail & Elderly Population | Community-Based Senior Care

Position Overview

A growing, established PACE program in Northern Indiana seeks a Board-Certified Family or Internal Medicine physician with geriatric experience to serve as Medical Director . This leadership role offers the chance to guide an interdisciplinary team and make a meaningful difference in the lives of vulnerable seniors.

Role Highlights

Provide clinical leadership across clinic, home, and community-based care

Supervise and mentor Primary Care Providers and APPs

Lead Quality Assurance & Performance Improvement (QAPI) initiatives

Develop medical policies, protocols, and care standards

Participate in care plan meetings, intake evaluations, and family conferences

Perform new and follow-up physical exams and assessments

Represent the program at community and partner meetings

Shared provider on-call responsibilities

Compensation & Benefits

Salary : $231,000–$280,000 (based on experience)

Candidate Qualifications

BC in Family or Internal Medicine – Required

BC or CAQ in Geriatric Medicine – Highly Preferred

Indiana medical license or eligibility – Required

DEA, CDS, and BLS – Required by start

At least 1 year working with frail or elderly populations – Required

Prior supervisory / leadership experience – Required

Proficient in Microsoft Office & EMR systems

Valid driver's license, vehicle, and insurance
