The Nurse Practitioner provides health care services to patients within scope of license, performs physical examinations and diagnostic tests, assessments and screenings, prescribes medications, and instructs patients and family members on medical conditions and treatment. Additionally, the Nurse Practitioner may have an area of specialty practice.
Minimum Requirements :
Current Nurse Practitioner Certification and license in Indiana
Certification as a Nurse Practitioner by a nationally accredited nursing organization (e.g. American Academy of Nurse Practitioners - AANP, American Nurses Credentialing Center - ANCC) is preferred
Must be a Family Nurse Practitioner
Must have a minimum of 1 year of primary care NP experience - must have experience working in a primary care outpatient clinic setting with adult and geriatric patient population
Must have an active Indiana DEA
Maintain collaboration agreements, per state requirements
Current BLS if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Must be at least 18 years of age
