Summary : Manages and oversees the DAS Case Management and Support at home Program.Providing case management services to seniors and adults with disabilities.
Essential Functions :
- Responsible for the planning, development and implementation of the Case Management and Support at home Program, as well as quality services of the Social Services Program.
- Assists with overseeing related programming and analyzes program performance data; ensures service quality and meeting program goals. Identifies areas for improvement and recommends changes as needed.
- Trains, supervises, and evaluates the program's staff.
- Provides case management services to seniors and adults with disabilities, which include :
- a. Conducting intake;
- b. Completing a comprehensive assessment of the consumer's functional and cognitive capacity and limitations during a home visit;
- c. Developing a care plan based upon the assessment;
- d. Implementing the care plan by coordinating and arranging for services;
- e. Following up and monitoring the plan to ensure that the care plan continues to meet the consumer's needs;
f. Reassessing the client's needs on a regular basis; and
g. Terminating case management services when appropriate.
h. Maintain confidentiality and adhere to HIPAA and organizational privacy policies.
Maintains complete client files and completes all required data collection forms and reports.Participates in evaluation and quality assurance activities to determine the effectiveness of the Social Services programs as required.Assumes responsibility for own professional growth and continuing education to upgrade case management skills; provides input to the Manager / Director in developing the training plan to meet individual as well as the case management staff's training needs.Participates in case conferences, in-service training and other meetings as required.Networks with and conducts outreach to community-based organizations, other service providers, and families and caregivers; attends and makes presentations at community meetings and other appropriate venues to inform the community about the agency's services.Performs other duties as assigned.Qualifications :
Bachelor's degree in health, social work, counseling, psychology, gerontology, or related fields with extensive case management experience and some administrative / supervisory experience. A master degree is strongly preferred.Knowledge, skills, and experience necessary to assess the consumer's needs for services and to perform the core functions of case management.Sensitive to the needs of the elderly and disabled adults from diverse ethnic and minority cultures.Excellent communication and interpersonal skills.Must be organized and able to multitask.Able to work independently and in a team setting.Bilingual and bi-literate in English and Chinese.Must be able to travel via public transportation or personal vehicle for the performance of job duties.
