Program and Division Description

Felton’s Senior Division specializes in a full continuum of mental health services for older adults in outpatient clinics, the community, and supportive housing sites, as well as programs in employment training, economic security, case management, advocacy, and social connection and inclusion.  1633 Valencia is a brand-new permanent supportive housing site for formerly homeless older adults in the Mission Neighborhood. Our program will provide welcoming, culturally congruent, and trauma-informed services to formerly homeless, income-eligible older adults aged 55+ residing at 1633 Valencia Street in San Francisco. Tenants are referred to through the Department of Homelessness and Supportive Housing’s Coordinated Entry and Housing Ladder programs. The Support Services team offers comprehensive case management focused on community building, housing retention and stability, connecting residents with community resources, benefits advocacy, health and wellness services, and social programming. Staff work closely with the Mercy Housing property management team, conduct wellness checks, resolve housing issues, and engage tenants in social groups and community building, while promoting a harm reduction approach and ensuring services are delivered equitably and inclusively in alignment with Felton’s mission.

Job Description

Under the supervision of the Director of Workforce and Engagement Services, this position leads a team of case managers and peer support specialists to serve tenants of the 1633 Valencia Apartments. The Program Supervisor coordinates the engagement, assessment, case management linkage, and housing stability services. The Program Supervisor will coordinate closely with Mercy Housing, our property management partners, wellness checks, and emergency safety checks. The Program Supervisor will also coordinate the monthly group schedule and community meeting.  This leadership position provides structure and program development in operations, in collaboration with the Director of Workforce and Engagement Services, and provides supervision to the three case managers and two peer support specialists on the team.

Program Specific Responsibilities

• Responsible for overall operations of the Mercy Housing contract, under the direction of the Director of Workforce and Engagement Services in providing housing stabilization and engagement services in collaboration with Mercy and Housing and Supportive Services (HSH).
• Collaborative and coordinated team efforts with Mercy Housing staff and all internal and external stakeholders
• Supervises the interdisciplinary team onsite to provide onsite support to residents
• Creates a welcoming community milieu within the apartment building by supporting meal programs, community building events, and tenant advocacy.
• Acts as a point person in collaborating with property management and HSH to support tenants in maintaining stable independent housing
• Attends appropriate meetings with building partners, funders, and external partners, as directed by the Director of Workforce and Engagement Services.
• Provides crisis intervention and management to prevent eviction and hospitalization, and if necessary, collaborates with the supportive linkage teams as required.
• Provide skills-building and case management services based on medical and resource linkage needs.
• Maintains thorough records in accordance with mandated and compliance standards from Housing and Supportive Services (HSH).
• Oversees the documentation needs of the case managers and peer support specialists, building capacity and expertise within the team.
• Leads the team meetings and case conferences.  Exchange of resources and ideas, including hosting training courses in collaboration with Mercy Housing staff.
• Manages data collection in the ONE system in collaboration with HSH and Mercy Housing.
• Participates in Felton’s training opportunities, including the development of specializations in evidence-informed treatments and practices, is an ongoing training approach.
• Other duties and responsibilities as assigned

FLSA Status : Exempt

Salary Range : $105,000 - $115,000 Annually (DOE)
