[job_card.job_description]One year experience in social / medical services setting working with homeless individuals AND have a social work / mental health related Bachelors’ degree At least two years of experience providing direct mental health or intensive case management services; Comfortable working with diverse populations including formerly incarcerated individuals, undocumented individuals, LGBT populations, communities of color, and those experiencing homelessness, substance abuse, and / or severe mental illness; Working knowledge of and experience engaging and assisting individuals with chronic / complex medical conditions, such as HIV, HCV, SMI, SUD, to access / navigate health care as well as other needed services; Experience with Housing First and Harm-Reduction strategies, Motivational Interviewing, Patient Centered and Trauma Informed Care, Crisis Intervention & Prevention strategies; Experience working with Clarity and Coordinated Entry System; Bilingual English / Spanish required (read, write, speak) Carry a caseload of up to 20 clients; Successful completion of DHS-required workshop series; Conducts intakes and enrollment activities to establish program eligibility, individual patient needs and goals; Assess self-management skills of individual patients; promote and aid patient in establishing self-management skills; Care plan development for patients including goals, objectives and actions to resolve barriers and access to services; Empower and educate client on tenant rights and responsibilities, eviction prevention, appropriate use of crisis intervention services vs. 911 emergency calls, maintaining medication and treatment adherence; Assist clients in obtaining income and / or establishing benefits, locating employment and / or educational opportunities, obtaining basic needs, such as clothing and food, budgeting and money management, life skills and community participation and building the skills and capacity needed to successfully navigate healthcare and social service systems;; Maintains consistently accurate records, client tracking data, and monthly reports, including appointments, services, follow-ups and assessments in multiple databases, as dictated by St. John’s and Department of Health Services; Chart case management notes in CHAMP, Clarity, and St. John’s electronic medical records (creating alerts for providers, as needed); Work with medical providers to determine health priorities and develop appropriate care management plans; Attend staff meetings, case conferences, team meetings and trainings as required or assigned; Other duties may be assigned or may be modified as business needs dictate

This position will provide high quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the LA County Department of Health Services (DHS) Housing for Health (HFH) program. A client-centered approach will be used to provide excellent customer service that is sensitive to the challenges that people experiencing homelessness with a range of medical and behavioral health issues face, while employing a “whatever it takes” approach to assist clients in their transition from homelessness to permanent housing.
