Jefferson Healthcare is seeking an on call, per diem Medical Social Worker to join our Emergency Department Team. In this role, you will deliver short-term services to support patients and families in managing personal, financial, psychosocial, and environmental challenges related to the patient's medical or psychiatric conditions by connecting them with valuable community resources. Our Emergency Department and Case Management team are collaborative groups committed to serving our patients and community.
Schedule : Per Diem; 0-16 hours / week; Variable Shift; 24 / 7 on call

What you'll need :
Washington State Licensed Clinical Independent Social Worker (LICSW)Two (2) years recent LCISW experience in health care settingMinimum two years Social Worker experience in healthcare setting, clinical social work in similar settingExperience in working with specific patient populations that may include but are not limited to : children, frail elderly, vulnerable adults, and patients impacted by mental illnessPrevious experience in AIMS collaborative care primary care clinic is a plusMay require valid Washington state driver's licenseTo apply : visit our careers website at https : / / jeffersonhealthcare.org / healthcare -careers /
