What You’ll Do

• Support residents through psychosocial assessments and individualized care planning

• Help residents and families navigate the transition to long-term and memory care

• Complete MDS-related documentation and maintain accurate, compliant records

• Advocate for resident rights, dignity, and self-determination

• Coordinate discharge planning and community resources

• Collaborate with nursing, administration, and the interdisciplinary team

• Assist with survey readiness and regulatory compliance

What We’re Looking For

• Certified, licensed, or registered in the state of practice (required)

• Bachelor’s degree in Social Work or a related Human Services field

• At least 1 year of social work experience in a healthcare or geriatric setting

• Skilled Nursing Facility experience preferred

• Nursing experience in a SNF a plus
