Case Manager Blended Schedule (up to $3640/month)

One of our clients is seeking Case Managers to provide services in the Philadelphia area.

CLIENT'S AVAILABLE HOURS
8:30 am- 5:00 pm
Multiple client-based quarterly incentives.

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Coordinate the performance of initial and ongoing evaluations of the child and family.
* Complete initial and ongoing assessments of the child and family as required.
* Assist families in identifying early intervention services and support identified on their IFSP
* Enter service notes the same day that the activity occurs.

CLIENT'S REQUIRED SKILLS & EXPERIENCE
* Bachelor Degree under Human Services umbrella (psych, social work, sociology, criminal justice, political science.
* 1 year experience working with children
