Experience : Minimum of one year experience in Long Term Care / Home Care setting as Physical Therapist, recent home care experience within the last year
License : Current state license in Physical Therapy required
Certifications : BLS (AHA ONLY)

Description :
Provides case management and direct patient care within a team environment.
Administers comprehensive physical therapy modalities in the home setting.
Assists the physician in evaluating level of function.
Helps develop the plan of care, revising it as necessary.
Prepares clinical and progress notes.
Advises and consults with the family and other personnel.
Participates in in-service programs.
Ensures Care Delivery Services are consistent with the organization's vision, mission, values, and strategic goals.
Promotes effective working relationships and works effectively as part of the team.
Utilizes resources, supplies, and time efficiently to contain costs and increase service value to the community.
Promotes positive, supportive, and respectful communication to all external sources, patients, families, and staff.

Implements home therapy programs, including gait training, transfers, activities of daily living, home modifications, and equipment use.
Periodically evaluates patients' progress and makes necessary adjustments to therapy programs.
Instructs para-professional staff in body mechanics, equipment use, bed mobility, transfers, and home exercise programs.
Coordinates, evaluates, and supervises quality of patient care services provided by the home health agency, including clinical supervision of home health aides.
Interacts with patients, physicians, referral sources, and others in a manner conducive to positive relationships.
Displays an exemplary level of patience, courtesy, and tact.
Supports care management philosophy through collaborative planning and case management designed to meet individual service needs.
Participates in regularly scheduled patient conferences for discussing patient care and comprehensive treatment plans.
Assists in orienting new rehabilitation personnel.
Completes all appropriate clinical records for compliance with legislation and service delivery timelines.
Refers to appropriate community agencies and maintains current knowledge of available services.
Demonstrates flexibility, enthusiasm, and willingness to cooperate while working with others.
Ensures problem resolution at the point of contact.
Makes constructive recommendations to improve quality of patient care and operations.
Maintains integrity and ensures patient needs are met throughout the delivery process.
Ensures authorization and release of information forms are signed by patients or responsible parties.
Meets performance competencies and participates in achieving operational and financial objectives.
Monitors quality indicators, including customer satisfaction with the Physical Therapist function.
