Summary :
Join our dedicated team as a Care Manager RN, where you will work collaboratively with physicians and interdisciplinary teams to facilitate positive health outcomes for clients. You will assess, plan, implement, coordinate, and evaluate services to meet the unique health needs of individuals. Your skilled communication and educational resources will be instrumental in promoting quality, cost-effective care.
Responsibilities :
- Utilize critical thinking skills within the nursing process to assess and meet client health care needs based on research and evidence-based outcomes.
- Build collaborative partnerships with clients to explore their health care needs, choices, and the resources available to them.
- Advocate for, educate, and coach clients and their families about treatment options and community resources to promote self-care skills and independence.
- Enhance communication among health care team members and involve clients in decision-making to ensure cohesive service delivery.
- Demonstrate critical thinking, decisiveness, and the ability to operate independently in a fast-paced healthcare environment.
Requirements :
Graduate of an accredited nursing program. Preferred : Bachelor’s degree in Nursing.State licensure as a Registered Nurse (RN). Preferred : Basic Life Support (BLS) certification, Certified Case Manager (CCM).Minimum of 2 years of nursing experience.
