Salary Range: $50 to $60 per hour

Shift: 8-hour Day Shift (07:00 AM - 03:30 PM)

Duration: 13 weeks / 32 hours per week (Contract)

Job Description

Elitecare Medical Staffing is seeking a compassionate and skilled Travel Social Worker to join our team in Concord, CA. This role is vital in providing high-quality social services to patients and their families, ensuring their needs are met in a supportive environment.

Key Responsibilities

• Conduct assessments to determine the social, emotional, and financial needs of patients.
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Provide counseling and support to patients and families facing various challenges.
• Facilitate communication between patients, families, and healthcare providers.
• Assist in resource allocation and connect patients with community services.
• Maintain accurate documentation of services provided and patient progress.

Requirements

• Licensure/Certification:
• Current California BBS Certification.
• Basic Life Support (BLS) certification from AHA.
• Experience: Minimum of 2 years of experience in social work.
• Active CA Licensure: Acceptable candidates include LCSWs, ACSWs (MSWs), LMFTs, and AMFTs (MFTs) registered within California.

Additional Information

• Schedule: Will alternate weekly between Tuesday to Friday and Wednesday to Saturday.
• For Travel Nurse RATE: $60 per hr / $50 for Local *50 mile radius .
