Your Responsibilities - Deliver personalized, high-quality care to seniors in their homes, assisted living, hospice, and nursing facilities.
- Conduct comprehensive assessments and develop tailored care plans for older adults.
- Manage chronic conditions with skill and empathy for patients and families.
- Collaborate with cross-functional partners-including palliative, psych, and neuro-to deliver coordinated, whole-person care.
- Build trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals.
- Maintain consistency in documentation, follow-through, and patient communication.
What We're Looking For - Active, unencumbered Physician Assistant license in the state you will be practicing.
- Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
- Leadership potential or experience: Ideally, you've managed a panel successfully and are ready to grow into leadership roles.
- Experience in home-based care, SNF, or ALF settings strongly preferred.
- Compassionate, patient-centered approach and strong clinical judgment.
- Valid driver's license and reliable transportation.
