HWL Requisition : Riverwalk Sports Med - Outpatient Center-AN-0006 Physical Therapist : 8350 Riverwalk Park Blvd, Suite #2 & 3 Fort Myers, FL 33919. Shift : Monday-Friday 8 : 30AM- 4 : 45PM. 2+ years of PT experience required. Experience with simple neuro is great, should be comfortable with balance and vestibular, otherwise general ortho. FL license required and must be on profile. BLS required.\r\n\r\nRadius Rule 50 miles from 8350 Riverwalk Park Blvd, Suite #2&3 Fort Myers, FL 33919. FLOATING REQUIRED- Travelers are first to float, possibly float to other sister campuses as well. ALL RTO or SCHEDULE REQUESTS must be on submission- HCF will not be approving RTO or schedule requests after submission. (Please load all Demographic information at time of submittal : Address, email, full DOB, full SS, RN license number.) We will NOT be able to confirm anyone without this demographic information loaded in the profile. Culture is VERY important here. Travelers are welcomed into the units and are expected to function as core staff does. Hard document deadline is Monday prior to start. No exceptions for pending docs.
