REHABILITATION COUNSELOR - (FULL TIME)

DEPARTMENT : Single Room Occupancy (SRO) Program - Jamestown NY

SCHEDULE : Tuesday 9 : 00 am to 5 : 00 pm

Wednesday9 : 00 am to 5 : 00 pm

Thursday9 : 00 am to 5 : 00 pm

Friday9 : 00 am to 5 : 00 pm

Saturday 9 : 00 am to 5 : 00 pm

SALARY : Minimum $19.57 / hr.

AVAILABLE : ASAP

JOB FUNCTION :

Provide case management rehabilitative and therapeutic services to residents. Teach counsel and monitor residents activities of daily living in a community residence or apartment setting developing skills abilities and experience that will facilitate a greater opportunity for the resident to live in an independent environment.

MINIMUM QUALIFICATIONS :

Education Experience and Knowledge

Minimum education and experience : high school diploma / GED and customer service experience. Healthcare or behavioral health experience is a plus.

Preferred Education and Experience : Bachelors degree in a human services-related field or an associate degree in a human service-related field and three years experience in human services or a high school diploma / GED and five years experience in human service.

Must possess a working knowledge of current issues and concerns of people with mental illness rehabilitative training methods and community resources.

OTHER QUALIFICATIONS :

Have a home / cell telephone.

Be able to travel to and among various multi-level facilities with stairs only.

Have a valid drivers license and a safe driving record.

May be required to use personal vehicle for programmatic purposes.

The NYS Office of Mental Health NYS OMIG and other regulators require criminal background checks and other pre-employment checks.
