We are looking for a dedicated Physical Therapist to join the Community Clinic provider team. Our PTs are part of a multidisciplinary team of providers who provide exceptional, accessible care to patients of all ages. The ideal candidate will be able to work in both our Rogers and Springdale clinic locations and is comfortable working in a collaborative, patient-centered environment. The position is full-time, salaried, and benefits eligible.
Key Responsibilities
- Assess patients through evaluations and test results.
- Develop treatment plans in consultation with physicians or based on prescriptions.
- Administer manual exercises and physical therapy treatments.
- Instruct, encourage, and assist patients with exercises, functional activities, and daily living activities.
- Train patients in the use of assistive devices (e.g., crutches, canes, prostheses).
- Monitor and document patient progress.
- Adjust treatment plans as needed.
- Plan discharges in collaboration with providers, nurses, and other care team members.
- Design and teach home exercise programs.
- Educate patients, families, and caregivers on exercises and use of equipment.
- Recommend follow-up programs, assistive equipment, or outpatient / home health services.
- Maintain accurate and timely documentation in the electronic medical record.
- Meet productivity expectations.
- Practice within scope of licensure.
- Participate in continuing education and maintain licensure requirements.
- Use clinic resources to support patient outcomes.
- Support and promote the mission of Community Clinic.
- Adhere to laws and regulations, including but not limited to HIPAA / HITECH, CLIA, HRSA, NCQA PCMH and OSHA.
