TAG MedStaffing is hiring an LPN Care Coordinator (Remote) for our client based in San Francisco, CA. This primarily remote role supports chronic care management through telehealth outreach, care coordination, documentation, and patient engagement. You will use EMR and digital health platforms to maintain CMS-compliant care plans while collaborating with RNs, neurologists, caregivers, and external providers. This is a fully remote role with rare on-site visits and requires candidates to reside in California, ideally near the San Francisco Bay Area.
Responsibilities as the LPN Care Coordinator (Remote) :
- Care Coordination : Provide ongoing care coordination, follow-up, and support for patients in chronic care management programs.
- Telehealth Engagement : Conduct virtual and phone check-ins to monitor patient status, reinforce care plans, and encourage adherence.
- Care Plan Documentation : Assist RNs with updating care plans and documenting interventions in the EMR.
- Interdisciplinary Communication : Coordinate with physicians, caregivers, and clinical teams to support timely updates and continuity of care.
- Administrative & Clinical Support : Manage scheduling, documentation, data entry, and escalate clinical concerns to supervising RNs when needed.
Qualifications for the LPN Care Coordinator (Remote) :
Licensure : Active, unrestricted California LPN license required (multi-state licensure preferred).Experience : Prior experience in chronic care management, care coordination, or telehealth, particularly working with older adult or neurodegenerative patient populations.Technical Skills : Proficient with EMRs, digital health platforms, telehealth tools, and remote documentation workflows.Industry Knowledge : Understanding of CMS PCM / CCM program requirements, quality improvement practices, and interdisciplinary communication standards.Skills & Attributes : Empathetic, organized, and proactive communicator with strong problem-solving skills and the ability to manage a remote patient caseload effectively.Application Notice : Qualified candidates will be contacted within 2 business days of application.
