Job Overview – LPN Care Coordinator (Remote) :

Compensation : $60,000 / year

Location : San Francisco, CA

Schedule : Monday to Friday (Remote)

TAG MedStaffing is hiring an LPN Care Coordinator (Remote) for our client based in San Francisco, CA. This primarily remote role supports chronic care management through telehealth outreach, care coordination, documentation, and patient engagement. You will use EMR and digital health platforms to maintain CMS-compliant care plans while collaborating with RNs, neurologists, caregivers, and external providers. This is a fully remote role with rare on-site visits and requires candidates to reside in California, ideally near the San Francisco Bay Area.

Responsibilities as the LPN Care Coordinator (Remote) :

• Care Coordination : Provide ongoing care coordination, follow-up, and support for patients in chronic care management programs.
• Telehealth Engagement : Conduct virtual and phone check-ins to monitor patient status, reinforce care plans, and encourage adherence.
• Care Plan Documentation : Assist RNs with updating care plans and documenting interventions in the EMR.
• Interdisciplinary Communication : Coordinate with physicians, caregivers, and clinical teams to support timely updates and continuity of care.
• Administrative & Clinical Support : Manage scheduling, documentation, data entry, and escalate clinical concerns to supervising RNs when needed.
