    IV Teams, Physical Therapy, Respiratory Services, Interpretation Services, Phlebotomy, Radiology, Social Services, Rapid Response Teams, Pharmacy, Transportation, Unit Secretary
  • Patient Diagnoses:
    REQUIRED:
    * Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, POLST
    * Ability to organize and participate in family meetings.
    * Participate in the development of the treatment plan.
    * performing both roles of Care Coordination and

    AIDA, PASRR
  • Special Procedures/Unit Details:

    Performing role of care coordination and discharge planning
  • Special Equipment:
    Computerized documentation, is required.
    2-year acute hospital case management experience required

    - Will be required to watch up to 2 units on the weekend and focus on discharge planning
