Description :

This is a full-time position working Monday through Friday from 8 : 00 AM to 5 : 00 PM.

Mileage reimbursement is provided.

Home health experience is a requirement.

The role involves covering a specific service area.

Planning and conducting segments of treatment programs designed to restore and improve physical, social, and mental functions are essential responsibilities.

The position requires serving as a co-leader in an interdisciplinary approach to patient groups.

Individualized treatment plans for patients must be established.

Comprehensive and accurate clinical documentation within established deadlines must be completed.

Supervision of assigned assistants is part of the role.

Other job-related duties may be assigned as needed.
