Job Summary

We are seeking a dedicated and skilled Occupational Therapist to join a Home Health team providing high-quality care to patients in the Sacramento area. This role focuses on delivering patient-centered therapy services, managing documentation within required timelines, and maintaining clear communication with supervisors and office staff.

Key Responsibilities

  • Prioritize and complete Start of Care (SOC) visits.
  • Supervise Occupational Therapy Assistants as needed.
  • Submit next-day reporting for all Start of Care visits.
  • Complete clinical notes and documentation within 48 hours of each visit.
  • Maintain ongoing communication with supervisors, intake coordinators, and office personnel.
  • Remain responsive and available during regular business hours.
  • Provide therapeutic services according to the patient's plan of care.
  • Attend mandatory training (3 full days : 1 in-house, 2 field-based).
  • Travel to patient homes within the assigned territory.

    • Requirements

  • Active California Occupational Therapist (OT) License .
  • Minimum 1 year of Home Health experience .
  • Ability to manage productivity expectations (30 points per week).
  • Reliable transportation for travel between patient visits.

  • EMR System : Kinnser
  • Equipment Provided : iPad and RingCentral access
  • Mileage Reimbursement : $0.655 per mile (first to last patient)
  • Parking and tolls : Reimbursed

    • Productivity Structure

  • 30 points per week required
  • Visit Point

    • Applications will be accepted only from candidates who meet the minimum qualifications and hold the necessary certifications.
