Job Description : Acclaim is seeking a Full-Time Advanced Practice Provider to join our Infectious Disease / HIV team. The Department of Internal Medicine provides specialty services across the continuum of care at JPS. We currently have over one hundred providers who work in various locations including the Main Hospital, Ambulatory Surgery Center, Community Medicine Clinics, and a robust Street Medicine program.

This position is grant funded and subject to re-evaluation based on program changes.

The Department of Internal Medicine utilizes a robust APP onboarding program, inclusive of a 6-8 week training phase in which APPs can safely learn and drill the fundamentals of patient care, before their official patient templates open. Furthermore, this onboarding includes a thoughtful and well-calibrated template ramp up, such that APPs have the time to learn and grow appropriately in their new role.

Job Summary :

  • This position is an outpatient service for routine care for those patients who are HIV+ or at risk for HIV and any related health issues consisting of assessment, diagnosis and the development and implementation of patient-specific treatment plans for patients and their families.
  • While the schedule is typically Monday-Friday, the exact dates and times will be developed in conjunction with the candidate and the needs of the program.
  • The clinician will order and interpret laboratory and diagnostic tests, as well as consult with other members of the healthcare team when appropriate. Additional services include providing cervical cancer screenings and assisting / performing high resolution anoscopies.
  • The candidate will work with Healing Wings HIV clinic staff and JPS Community Health as needed to support the Healing Wings outreach program. Emphasis is placed on providing treatment where patients are located through pop-up clinics, community clinics, True Worth Shelter clinic, or other locations where patients may be located and need targeted outreach.

  • Independently and in collaboration with physician(s) and interdisciplinary team members, provides comprehensive clinical and organizational direction and leadership to the healthcare provider team in the care of patients.
  • Performs comprehensive history and physical assessment.
  • Documents findings and noting pertinent normal and abnormal findings.
  • Orders and interprets the appropriate diagnostic labs and tests.
  • Forms medical diagnoses based on history, assessment, and diagnostic findings.
  • Develops, implements, and evaluates treatment plans.
  • Prescribes and recommends pharmacological and non-pharmacological interventions.
  • Initiates appropriate specialty consultations referrals when requisite.
  • Initiates and evaluates clinical pathways and protocols.
  • Collaborate with Attendings and / or Fellows regarding admissions and community transfers.
  • Facilitates the timely flow of patients via collaboration with interdisciplinary team members and supports all interdisciplinary relationships.
  • Recognizes acute / emergent situations requiring immediate responsiveness and initiates life-saving interventions, alerting the interdisciplinary team members for assistance.
  • Recognizes and incorporates age-specific requirements, communicates plan of care with patients and families.
  • Advocates for patients and family members in educational needs, treatment options, participation in care and ethical issues and patient rights with recognition and consideration of the diverse needs of the individual / family.
  • Provides consultation in specialty areas.
  • Maintains confidentiality of patient information.
  • Coordinates meeting applicable clinical standards for all disciplines in collaboration with interdisciplinary team members.
  • Explains the necessity, preparation, nature, and anticipated effects of procedure(s) to patients, patient families, staff, and students.

  • Duties and Essential Job Functions :

  • Coordinates the multidisciplinary care delivered to all patients, to optimize patient care.
  • Reviews patient medical charts to diagnose, treat, and provide care management and education to patients and families.
  • Makes appropriate patient referrals as necessary.
  • Monitors guidelines for the management of care and for ensuring the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.
  • Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Demonstrates patient centered and patient valued behavior.
  • Maintains a collaborative environment with other referring physicians, residents, clinical staff, and support staff to provide quality patient care.
  • Supports academic, educational and research endeavors.
  • Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.
  • Partners and communicates with acumen to build a positive brand and patient / customer experience for those who encounter JPS Health Network and Acclaim Physician Group. This would include health network staff, vendors, outside partners, patients, stakeholders, and local community leaders.
  • Serves as a resource to the community.

  • Qualifications :

    Education and Experience :

  • Master's Degree in Nursing from an accredited school of nursing OR Graduate of an accredited Physician Assistant Program
  • Licensure / Certification / Specialized Training :
  • Must have current Texas Physician Assistant license or Texas APRN license (FNP or AGPCNP)
  • Basic Life Support (BLS) certification1 Year Infectious Disease / HIV experience preferred.
