Job Description
We are seeking Nurse Practitioners / Physician Assistants with Full Practice Authority to perform in-home / telehealth health assessments for patients in the Chicago area. This is a unique opportunity to work with patients who may be less engaged with traditional healthcare, helping them better understand and access the care they need.
What You’ll Do :
- Conduct comprehensive health assessments in patient homes or via telehealth.
- Review and update medical histories and current medications.
- Document findings and care plans in EHR.
- Identify health risks, chronic conditions, and gaps in care.
- Travel within Chicago as needed (mileage reimbursed).
