We serve residents ages 18 and above, many of whom live with complex mental health needs, chronic illness, or developmental disabilities. Our psychiatric nurse practitioners partner closely with each facility's long-term care healthcare team to deliver individualized, high-quality mental health care for residents. Responsibilities may include :

• Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
• Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
• Educating and supporting caregivers and facility staff.
• Managing and adjusting psychiatric medications to promote stability and well-being.
