Overview

Coordinated Care Agency offers comprehensive services, enhanced shelter, and housing for people experiencing poverty and homelessness. Our services include permanent supportive housing, enhanced shelter, rapid re-housing, tenant-based voucher programs, outreach and engagement services, day centers, and health services. The purpose of our work is to ensure people have access to the housing, healthcare, and support they need to thrive. The team works to promote social changes to dismantle systemic racism, injustices and inequities that contribute to housing instability.

Our mission is to provide a hospitable, low‑barrier program grounded in a housing‑first model. We operate in the spirit of the ministry of presence, fostering individualized attention and respect for each person’s unique circumstances. Our programs serve highly vulnerable individuals who may have significant needs, including severe physical disabilities, chronic or acute mental illness, active chemical dependency, and experiences of trauma. We focus on helping participants obtain or maintain permanent housing through consistent, supportive case management. CCA is committed to ensuring that our services, our agency, and our communities are free from the dehumanizing impacts of racism. We strive for anti‑racist practices and programming by advancing racial equity and justice in our daily work.
