Position Overview:
We are seeking a caring, reliable, and motivated Licensed Practical Nurse (LPN) to join our Home Health Nursing team. As an LPN in home health, you will provide direct patient care, support care plans designed by RNs and physicians, and bring comfort and clinical expertise directly into patients' homes. Responsibilities:

• Provide skilled nursing care to patients in their homes under the supervision of an RN.

• Monitor and document patient conditions, vitals, and progress notes.

• Collaborate with the care team (RNs, physicians, therapists) to ensure quality outcomes.

Qualifications:

• Current Licensed Practical Nurse (LPN) license in Indiana.

• Previous nursing experience preferred; home health experience a plus (training available).
