Job Summary
As an LPN in a primary care setting, you will provide essential nursing care under the supervision of a provider, working collaboratively with a dedicated team to ensure the best patient outcomes. Your role is vital in a non-acute ambulatory setting, following the State Board of Nursing regulations, professional standards, and Prisma Health's core values.
Essential Functions

- Deliver personalized patient care as delegated by the Registered Nurse or Provider, addressing physical, emotional, spiritual, and educational needs.
- Assist providers during examinations and procedures to enhance patient care experiences.
- Coordinate patient appointments for necessary testing, referrals, and hospital admissions seamlessly.
- Monitor and maintain medical supplies and ensure all equipment is in perfect working condition.
- Accurately document patient statistics and encounter information in charts, while gathering medical histories and chief complaints.
- Promote positive health behaviors through effective patient education efforts.
- Prepare examination rooms with the essential instruments and supplies for optimal care delivery.
- Maintain cleanliness of exam rooms between procedures to promote a safe environment.
- Handle clinically related calls, screening and forwarding messages for provider action as needed.
- Complete medical history forms for insurance and document necessary information.
- Administer medications and treatments as mandated by a provider, ensuring adherence to nursing protocols.
- Be a role model and mentor, sharing knowledge with fellow team members.
- Participate actively in departmental and organizational meetings to stay informed and engaged.
- Complete all required compliance training annually and as necessary.
- Utilize supplies and resources efficiently and safely.
- Ensure a clean, safe, and caring environment for all patients and staff.
- Invest in your own professional development and continuing education opportunities.
- May take on additional roles such as preceptor or unit champion as needed.
- Adhere strictly to compliance, policies, and procedures to ensure quality care.
- Perform other duties as assigned to support team objectives.
Supervisory / Management Responsibility
This is a non-management position that reports directly to a supervisor or manager.Minimum Requirements
Education - Completion of an LPN program recognized by the State Board of Nursing.Experience - No prior experience required; new graduates are welcome to apply.In Lieu Of

Current LPN compact / multistate license recognized by the NCSBN or state licensure where you will work.If obtaining RN licensure, you may continue in this role temporarily until placement in an RN position.Knowledge, Skills and Abilities
