TLC Nursing Associates, Inc. is seeking a skilled and compassionate Registered Nurse (RN) to provide high-quality care to patients in a home health setting. The ideal candidate will deliver personalized nursing services, monitor patient progress, and ensure continuity of care in collaboration with physicians and interdisciplinary teams.

Job Responsibilities

• Conduct in-home assessments to evaluate patient health status and develop individualized care plans.
• Administer medications, wound care, and other treatments as prescribed by the physician.
• Monitor patient conditions and update care plans based on changes in health status.
• Provide education to patients and caregivers on disease management, medications, and safety practices.
• Document all nursing activities accurately and in a timely manner according to agency and regulatory standards.
• Coordinate care with physicians, therapists, social workers, and other home health staff to ensure optimal outcomes.
• Follow infection control procedures and maintain a safe care environment in the home.
