Michigan Licensed RN Case Manager

A company is looking for a Remote RN Case Manager with Michigan Licensure.

Key Responsibilities

Manage a caseload of members, facilitating high-quality, cost-effective patient outcomes

Educate members on kidney health, dialysis, and transplant options while engaging them in the Kidney Health Program

Coordinate care among patients, healthcare providers, and support networks, ensuring compliance with regulations and policies

Required Qualifications

Active, unrestricted RN license required

Bachelor's degree required

CCM certification preferred

Three (3) years of experience in case management preferred

Experience in a dialysis or transplant center preferred
