Job Title : Physical Therapist - Home Health

Must-Have : Job Description :

• Prioritize start of care visits and supervise Physical Therapy Assistants.
• Ensure next day mandatory reporting of start of care visits.
• Complete clinical notes and other documentation within 48 hours of the visit.
• Maintain timely communication with supervisor, intake, and office personnel.
• Remain generally responsive during normal business hours.
• Require 40-hour work weeks, Monday-Friday, with one weekend rotation per month.
• Undergo three full days of training, including one in-house and two in-field.
• Utilize Kinnser documentation system.
• Use provided iPad and communication equipment.
• Participate in productivity requirements of achieving 30 points per week, with points reflected through various visit types.
• Caseloads and visit mixes per day will equal an average of 8 hours per day and 40 hours per week.
