Actual amount subject to eligibility and seasonal / annual adjustments

1 : 1 Patient care with one 45 min appointment every hour.

Performs appropriate evaluation tests and measurements based on the patient’s diagnosis.

Develop treatment plans, including reassess and revise every 6 weeks or earlier based on patient's progress.

Performs physical therapy techniques appropriate to the diagnosis.

Demonstrates use of physical therapy modalities appropriate to the diagnosis.

Documents progress reports for patients in a timely manner to coincide with physician follow up visits, and to assist with compliance for insurance carriers and utilization review companies for authorization and reimbursement.

In addition to scheduled weekday and Saturday shifts, will share with other PTs coverage for in-patient care on holidays and Sundays (generally one holiday per year and Sunday coverage 6-8 times per year).
