[job_card.job_description]Pay Range : $73,000-132,000 annually Annual Bonus Incentive : Eligible for 8% bonus Conduct UAS-NY assessments for long-term care members in home or community settings Evaluate service needs of complex / high-risk members and develop personalized care / service plans Assess ongoing needs and collaborate with care management teams, providers, and community resources Monitor care plan progression, clinical changes, and goal achievement Coordinate services, treatments, and resources to ensure member-centered care Educate members and caregivers on care options, disease processes, benefits, and referrals Document all assessment and care management activities in compliance with state, federal, and clinical guidelines Act as a liaison between members, families, providers, and agencies Potentially precept and mentor new clinical hires Participate in improving care quality and service delivery Active NY RN - Registered Nurse - State Licensure and / or Compact State Licensure required Residency in Onondaga County to cover Onondaga and surrounding counties of Oneida, Madison and Cortland UAS Experience; Ability to assess and coordinate medical and supporting services Experience in care management, preferably in a Managed Care Organization (MCO)
