    DETAILS :

  • Role will be at our newly built outpatient facility starting in Sept 2025
  • Connected to our independent living facility
  • Patient-focused, relationship-driven care model
  • Support staff in place to help you with all other duties
  • Weekly physician visits and easy access to physicians via phone
  • Open to full or part-time providers
  • Experience working with geriatric patients as an NP for 3+ years required
