Total Extended Care Services, a growing Physical Medicine and Rehab group is looking for a dedicated Nurse Practitioner / Physician Assistant to join our team. In this role, you will partner with our PM&R physician in the sub-acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.

About the Role :

Location : In-person at skilled nursing facilities

Schedule : Part-time position available

Culture : Detail-oriented, outcome-focused, stable, people-oriented, and team-oriented

Responsibilities : Partner with the facility's therapy department to provide the best possible rehab care. Communication and collaboration with the patient's primary care team to ensure the best outcomes. Timely charting / documentation is required but can be done from home.

Requirements :

Licensed Nurse Practitioner (Required)

Certified Nurse Practitioner (Required)

Licensed Physician Assistant (Required)

Certified Physician Assistant (Required)

Must be licensed in TN

Reliable transportation
