Role Overview

A mission-driven community health center in Santa Cruz County is seeking a Director of Health Information & Population Health . This leader will guide data, quality, and informatics initiatives for an 11-site system while maintaining a small outpatient clinical panel .

Key Requirements

Strong experience with EHR implementation or transition

Clinical informatics background or certifications highly valued

Spanish-speaking strongly preferred

No prior director-level leadership requiredopen to strong emerging leaders

Cannot sponsor visas

Compensation

Base salary : $270,400 $304,337.58 (DOE & FTE)

Sign-on / Retention Bonus : Up to $30,000

50% at start, 25% after year 1, 25% after year 2

Relocation : Up to $10,000
