Physical Therapist (PT) – 13-Week Contract

Location : Lawrence, MA 01841

Start Date : January 5, 2026

Contract Length : 13 Weeks

Shift : Day Shift – 40 hrs / week (Guaranteed Hours)

Setting : Skilled Nursing Facility (SNF) / Long-Term Care (LTC)

Job Summary

The Physical Therapist (PT) is responsible for evaluating, planning, and delivering evidence-based therapeutic interventions to help residents achieve maximum functional ability and independence. This role collaborates with caregivers, residents, and the interdisciplinary team within a skilled nursing / long-term care environment.

Key Responsibilities

  • Perform evaluative and therapeutic procedures under physician orders.
  • Complete resident evaluations and establish treatment plans with functional long- and short-term goals.
  • Incorporate resident and caregiver input into therapy goals and care plans.
  • Attend team meetings and coordinate care with physicians and interdisciplinary staff.
  • Monitor resident progress and adjust treatment plans as needed.
  • Document all evaluations, treatments, and education in the EMR according to facility standards.
  • Recommend appropriate assistive devices and provide usage education.
  • Educate residents and caregivers; document teaching outcomes.
  • Collaborate with and delegate appropriately to PT Assistants.
  • Ensure compliance with Medicare / Medicaid and reimbursement guidelines.
  • Provide in-service training and maintain clinical competencies.
  • Mentor new staff and support departmental program development.
  • Maintain cleanliness, follow infection control standards, and ensure equipment maintenance.

    • Requirements

    Education & Licensure

  • Bachelor’s degree required; Master’s or Doctorate preferred
  • Active PT license (MA – must be current)
  • BLS certification preferred

    • Experience

  • Minimum 3 years of experience
  • SNF / LTC experience strongly preferred

    • Skills & Competencies

  • Strong verbal / written communication and documentation skills
  • Excellent clinical judgment and analytical thinking
  • Ability to collaborate within an interdisciplinary team
  • Strong interpersonal and customer service skills
  • Proficiency in Microsoft Office
  • Resident-centered, compassionate care approach
  • Ability to read / write / understand English

    • Physical Requirements

  • Frequent standing, walking, bending, reaching, and lifting
  • Ability to lift up to 50 lbs (team lift required above 50 lbs)
  • Must be able to perform essential duties with or without reasonable accommodation.
